These are our service commitments:

1. Binders, MCS 90, certificates, ID cards, and accident packets to be delivered prior to inception of the policy.
    
2. Current loss runs provided quarterly.
    
3. All open claims reviewed monthly.
    
4. Safety consultations provided by insurance company at customer's request.  We will also work with the  customer regarding safety programs.
    
5. Annual safety survey provided by insurance company prior to renewal.
    
6. Review of exposures and market conditions conducted by American Agency with customer 90-120 days prior to renewal.
    
7. American Agency will provide evaluation of deductible and coverage options.
    
8. American Agency to provide evaluation of Ohio Workers Compensation exposures to determine if customer is eligible for the PIA Comp Management pool.
